Membership Appointments
Health Exchange Concierge offers both individual appointment options as well as yearly membership options that include unlimited follow up virtual visits. No more costly insurance copays or getting surprise bills from your primary care office because insurance failed to cover something.
IMPORTANT-Memberships are billed monthly and patients are required to agree to a one year commitment. If you select to book a membership office visit, the membership will be setup at the time of your first visit and payment will be collected for the first month, at the time of service.
Although we feel that our annual membership is the best option for the majority of our patients, we understand that it may not be right for everyone or that you may want to ensure that we are the right providers for you.
Each Provider will cap the number of members they see in order to deliver quality care to their patients and to ensure availability for our members to be able to schedule in a timely manner.
Non Member Appointments
INDIVIDUAL NON-MEMBER APPOINTMENT (IN PERSON )
- NEW PATIENT-60 Minute Appointment- Call for Pricing
All new patients who are looking to establish with us should select this option.
- Pre-Surgical Consultation- $250
If you need pre-surgical examination prior to surgery, please select this option. This includes an EKG and lab work, if necessary, which will be ordered and interpreted.
INDIVIDUAL APPOINTMENT (VIRTUAL)
- New Patient 60 Minute Virtual Appointment- Call for Pricing
All new patients who are looking to establish with us should select this option, if your first appointment is online.
INDIVIDUAL APPOINTMENT (IN HOME)
- Call for price-VARIES based on distance and provider availability. In-Home visits are not guaranteed and may be rejected due to availability.
