Erik Osborne
Meet Erik Osborne, a visionary leader and co-founder of Health Exchange Clinic and Journey Health.
In early 2023, driven by a commitment to patient well-being, Erik and his practice partner, Kristen Spratt, took a pivotal step: launching Health Exchange Concierge Medicine. After nearly six years in traditional family practice, Erik grew profoundly disheartened by the pervasive influence of insurance on patient care. Fueled by a passion for accessible, affordable, and functional healthcare, he pioneered a successful cash-based concierge model that now serves over 4,000 patients across the greater Charlotte area.
Erik’s unconventional path underscores his resilience and dedication to service. A Michigan native, he attended Michigan State University, where he walked onto the football team. When a career-ending injury curtailed his goal of becoming a military doctor, he found his calling in mental health. Erik earned his Master’s Degree in counseling in 2008 and served as a compassionate treatment director at St. Jude Home for Boys in Detroit, leveraging cognitive behavioral therapy to support abused, neglected, and delinquent youth.
His impactful work continued in academic settings, where he focused on goal-setting and mentorship for “at-risk” students. After relocating to North Carolina in 2010, he distinguished himself further as a dedicated counselor and therapist.
It was this rich career in counseling and leadership that led Erik to the Physician Assistant profession. He seamlessly credits his diverse background—spanning collegiate sports, deep mental health expertise, and educational leadership—with providing the unique perspective and confidence he applies today to transforming healthcare delivery, specializing in holistic, functional health.
Kristen Spratt
My name is Kristen Spratt, MSN, FNP, and I feel truly privileged to have a career I’m passionate about. Since beginning my practice in 2004, I’ve focused on functional, whole-person care and helping patients get to the root cause of their health concerns.
I’ve learned how important it is to really listen. Every patient has a unique story, and understanding that story allows me to treat the whole person—not just the symptoms. I value the meaningful connections I build with my patients and strive to create a space where they feel heard, supported, and empowered.
My passion for healing and wellness guides my work every day, and I’m committed to helping individuals restore balance and achieve lasting vitality
Tejal Patel
Dr. Patel graduated from one of the oldest and most prestigious medical schools in Canada, Queen’s University, in 2007 and subsequently has completed residency training in family medicine.
She has over 16 years of experience providing high quality, caring and compassionate healthcare to patients in comprehensive primary care with a special focus on women’s health and low risk obstetrics. In 2022, Dr. Patel obtained intensive hands-on training in aesthetic medicine including injectables and laser treatments before she made the move from Canada to Charlotte in 2023 to be closer to family.
She is excited to blend the best of both functional and traditional medicine as she partners with patients to help them achieve their health and wellness goals.

Vivian Pantelakos
Vivian Pantelakos earned her Bachelors of Science in Nursing from UNC Charlotte where she then also earned her Masters of Nursing as a Family Nurse Practitioner.
Prior to joining Health Exchange, Vivian was a Family Nurse Practitioner at CVS Minute Clinic in the South Charlotte area. As a nurse, Vivian has a combined 3 year experience ranging from pediatrics to elderly finding a passion for holistic medicine.
Using a holistic approach to primary care is vital in promoting overall wellness for the mind, body, and soul. With weight loss medication becoming a national craze, Vivian has done extensive research on the medical science and personal approach it takes to make weight loss successful and lasting.
Vivian is a native to North Carolina but loves all New England sports teams. She enjoys traveling to Greece to visit her family and exploring new areas of Charlotte.
